Membership Application Form

Please follow these steps to apply for ASID membership:

1. Read here to determine which membership category you wish to apply for.
2. Fill out the online form below and upload your curriculum vitae where indicated. You will need to indicate the names of two referees who are full ASID members who are willing to sign your application.
3. After submitting the form, you will need to print a hard copy of your application. Please ensure you print a copy. You will not be able to return to the form later.
4. Obtain signatures of your two referees on the hard copy of your application. Without these, your application cannot be considered by ASID Council. 
5. Post the completed hard copy form to: ASID, Suite 405, Level 4, 5 Hunter Street, Sydney, NSW 2000 or a scanned copy can be emailed to admin@asid.net.au or sent by fax on 02 9231 2907.

Do not send payment now. After your application is received and approved, you will be sent payment instructions by automatic email from the website. Your password will become active after your application is approved.
